I hereby allow Gubat St. Anthony Cooperative ("Cooperative") to provide me certain services declared in the forms where this Data Privacy Consent Form is attached.

As such I agree and authorize the Cooperative to:

1. Continue to use my existing information obtained before the signing of this Data Privacy Consent Form to process my loan applications, benefits, and claims;
2. Collect, record, organize, store, update, transfer use for background and credit investigation, monitoring and/or audit process as necessary, any information pertaining to my account;
3. Disclose such information (for official business purposes) to regulatory agencies pursuant to Court Information System Act, to third party partners, whether local or foreign. The Cooperative assures that security systems are employed to protect my information;
4. Use such information in the design, and communication of customer programs, marketing and promotional campaigns, promos, or offers;
5. Allow this Consent Form to remain valid from its execution and until 10 years after full redemption of my account/s, or at such time that I submit to the Cooperative a written letter of cancellation/ revocation of such Consent, whichever is earlier; I agree that my information will be deleted/ destroyed after this period.
